When patients come to our office seeking facial plastic surgery, many of them understandably focus on a single aspect of their appearance rather than the face overall. Blepharoplasty alone cannot always effectively treat sagging eyelids. Just as an endoscopic browlift can aid the upper eyelids, the midface lift can often enhance lower eyelid surgery to provide the younger, beautiful look that our patients seek.
Along with baggy eyelid skin, an additional source of concern is textural of surface changes, particularly fine lines, in the skin below the eyes. Although some improvement occurs with lower blepharoplasty, it does not address all of the superficial lines that result from premature aging, overexposure to sun and weather, and genetic predisposition. While lower eyelid surgery addresses excess skin and fat in the area, skin resurfacing techniques such as chemical peels and laser surgery address the textural wrinkles and imperfections.
In most cases, Dr. Chow performs his lower eyelid surgery via the transconjunctival approach, which involves an incision made just inside the lower eyelid with no external incisions. In specific circumstances, Dr. Chow will instead choose the subciliary approach, in which an incision just below the line of the eyelashes is made.
